Purpose of this assignment

1.    Understand the methods used when reporting and recording accidents and incidents (LO4)

You are a new trainee in an engineering company and part of your induction involved a short course on health, safety, risk assessment and welfare. The course includes accident/incident reporting, calculations of accident costs, etc.
Before you can start your training in the workshops, you need to demonstrate your clear understanding of health and safety with respect to accident and incident reporting and why it is necessary. You also should demonstrate your understanding of the more complex calculations of accident costs by completing the following tasks.
Task 1 – Part A – P7
State the main reasons why it is important to report accidents and incidents that occur in the workplace and also state when it is necessary to report accidents to the HSE.

Task 1 – Part B – P8
Produce and complete a sample accident/incident form with any relevant information that needs to be recorded. Also, describe the procedures that should follow an accident/incident e.g. calling first aider, etc. Also, clearly describe how to record serious accidents/incidents.
Task 2 – D2
In this task you need to determine the actual cost of an accident in terms of injury/disability, loss of life and money. To do this you need to focus either on the given case study below or a scenario or an actual accident in an engineering workplace. For this assessment you need to undertake your own research. Making extensive use of the internet to seek information on accident rates, incident and the cost of accidents. In your report you should state any assumptions or estimates you have made, and list the information sources you have used to produce your costing for the following:

1. Loss of his finger     OR
2. A fatality.

Case Study: Jordan Joseph
A small sheet metal and fabrication company with 20 employees is running on a tight schedule.
On one particular morning the drilling machine operator is absent and in order to ensure work output is maintained the manager tells Jordan Joseph a young inexperienced 24 year old engineer to operate the machine after giving him a brief demonstration.
Jordan suffered a horrific accident at work while operating the drilling machine. The little finger on his left hand was drawn into the rotating chuck, causing severe lacerations and a broken finger. As a consequence, Jordan’s finger had to be amputated. Jordan could have died from loss of blood.
The sheet metal and fabrication company was prosecute for breaches of the Health and Safety at Work Act and Jordan eventually received compensation for the loss finger.
